Forms Not Sending/Not Receiving Email Notification

I am receiving the messages on the forms listing page and the test email under settings comes through.

I fixed the line 40-46 issue in the debug, but I am still not receiving notification emails

I don't see it attempting to send the email. How do you have the form configured?

Really embarrassed, I hadn't checked the send email box, I had everything configured. Oh geeze. Feel free to delete the thread
