I reached mosre or less the same conclusions than you.
In my case, the tags disappears when the scheduled tasks run (when the post is published). Saving the post again, restores them and they stop disappearing from there on.
Maybe v4 will solve this issue?

Clive Walker : I am using Perch Runway 3.1.3. The opening <lastmod> element is present, but as it is empty, it shows up as <lastmod /> in my XML viewer (see screenshot of source code below).
dunc : the <perch:pages> element is closed with a trailing '/' before the '>'

I’m building a sitemap that includes the lastmod attribute.
While I have no problem including this info for blog entries, I’m stalling for regular pages.
The perch:pages namespace (Navigation and Pages) doesn’t provide access to the page modification date. E.g. <perch:pages id="pageModified"> doesn’t return anything.
I tried including a layout in the template which uses `perch_page_modified()` but that breaks the render of the page with the error “Namespace prefix perch on layout is not defined”.
Has anyone successfully built a sitemap with the lastmod attribute for pages?
I am running into a similar issue with the tags.
The tags ‘disappear’ from the site even though they are still present in the admin panel.
I tracked it down to entries being removed from ‘perch3_blog_posts_to_tags’ table each time the scheduled tasks are run. Saving the entry a second time solves the issue.

I'm running into a strange issue in Perch Runway:
Each time the scheduled tasks run and detects blog posts to publish, it remove the corresponding `perch3_blog_posts_to_tags` entries.
The blog entries retain their tags, but they aren't displayed until I save the entry again.
This only happens to 'new' posts. The next time scheduled tasks runs, these entries aren't affected.
Send/receive webmentions are turned on.
Any ideas ? I'm at a loss here…
